Additional Forms

In some cases, CSE and CAN will require additional forms in order to process your Activity Request Form. All the forms can be found on CSE's LUCommunity portal here.

One special form is the Liability Waiver Form. If your event involves any physical activities, you are required to fill in the Liability Waiver Form and submit a printed copy to the CSE office at least 24 hours before the event. Link to that form can be found here: RSO Liability Form 2020.
