Conduct Check Request Form
Student conduct records are maintained by the SRCR Team in the DOS for seven years from the date of the incident, with the exception of cases resulting in University expulsion (such files are retained indefinitely). Such files may contain contact information, correspondence, decisions, assigned outcomes, reports, and other information pertaining to any case in which a student was found responsible.
Student Request to View Their Own Record: Students have a right to view a redacted copy of their conduct file that falls within the seven-year period or resulted in University expulsion, with any protected information about other students removed, during regular business hours in the DOS suite or virtually via secure Zoom video platform. The SRCR Team requires a minimum of five business days’ notice to prepare files for review. In order to request to view a redacted copy of their conduct file, students must complete the Conduct Check Request Form (Permission to Release Education Information) that is linked below. Forms are reviewed in the order they are received and the SRCR Team will contact you to schedule a time for your meeting once your form is processed. Please note that copies of a student's conduct file information will not be distributed to the student.
Student Request to Release Their Record to a Third Party: In order to request a release of your conduct file information to another person or school/organization, students must complete the same form, Conduct Check Request Form (Permission to Release Education Information Form) that is linked below and if applicable, submit any associated form from the school/organization they want to receive their conduct file information. Forms are reviewed in the order they are received and are typically processed within 5-7 business days. The SRCR Team will typically issue a Dean's Certification letter along with any associated form in the event the request is for another school/organization.

Third Party Request to View Student Record(s): Loyola faculty/staff or verified third parties (employer or government agency) wishing to check the conduct record of a student in accordance with the Common App transfer admission process or to check the conduct record of one or more students for an award, scholarship, internship, background check, employment opportunity, honor society, organization, or University-led program (i.e. DC program or study abroad) may do so by completing the Loyola Third Party Conduct Check Request Form (Permission to Release Education Information) that is linked below. Loyola faculty/staff or verified third parties must submit proof of affirmative consent for release of records as well as provide the name of the student (single request or Common App request) or a a list of the students (batch check request) who have completed the release.
