- Request tickets for all DFPA ticketed programming
Final Exam Room Reservation Form
- Faculty only
- Complete this form if your class has an exam/class presetation during finals week and space is needed to host your exam/presentations
Key and Swipe Card Access Form
- Faculty and staff only
- Request for access to DFPA spaces, whether by key or by prox card/swipe.
- Please submit completed request forms two weeks in advance of anticipated delivery.
- Staff Only
- Requests for vacation, and other known scheduled time off that you would like to be “out of the office” (meaning not working, not working from an off campus location) for time off should be submitted using this form.
- This form can be used for known medical appointments, personal or vacation time or other time, it is emailed to both the DFPA Chair and Manageing Director. Once approved, it is requested that the staff member will assume responsibility of adding the details of their absence to the DFPA Outlook calendar.
Resource Requests
- This form can be used for requesting support, engaging guest artists that are Loyola employees or independent contractors, or for submitting additional required documentation including a Scope of Work or LUC Consulting Agreement.
- Depending on a variety of factors, processing time can range from 3-8 weeks. It’s encouraged to start the process early.

Office or Course Supply Request Form
- As a DFPA faculty, staff, or student worker who is not a departmental procard holder, use this form to request office supplies from Warehouse Direct to request faculty or student supplies for a course. If you have questions about the form, email DFPA@luc.edu. The main office handles the placing of orders and the follow up with requestors.
Professional Development Request Form
- Primarily intended for full-time faculty, but staff and part-time faculty are welcome to submit requests.
- Request funding for professional development, technological equipment or recruitment activities. Please see form for details.
- Proposals will be accepted on a rolling basis for opportunities occurring from June 1, 2022 - June 15, 2023.
- Please note that development funds are competitive and limited. Individual awards will be initially limited to $1500 each, typically not to exceed a maximum of $3000 total for all requests during this fiscal year. If funding remains after March 1st, applicants who had expenses that exceeded their awards may be invited to reapply for additional funding.
- Funds awarded to support recruitment activities or attendance at professional organizations on behalf of the department (NAST, NASM, ICFAD, etc.) do not count toward this total.
- Awards for professional development funding do not roll over from year to year and the funds awarded are associated with a specific request. Unused funding from one request is not to be applied to another activity. A separate request should be made for more funding.
- If you are awarded Professional Development Funds, to receive a reimbursement you will need to complete the DFPA Reimbursement Form.
- Unionized Faculty Members are encouraged to apply for the CBA Professional Development Funds established for unionized faculty member prior to seeking DFPA funding. The DFPA Request form can be used to submit the CBA Professional Development Fund application for Chair review.
- Primarily intended for CAS unionized faculty members (CBA17.1) (full- and part-time) who are no longer in the probationary phase of their employment (CBA 25.7)

- Fund is administered by the College of Arts and Sciences
- Applicants are eligible for up to $600 in funding per year to underwrite expenses associated, for example, with attendance to conferences, seminars, and workshops as well as engagement in pedagogically related professional development activities.
- Faculty and staff and approved students only
- If you are a member of the Department of Fine and Performing Arts and you have been approved for using departmental funds for purchases supporting professional develop, academic or public programming and need to be reimbursed, please submit this form.

Meeting Minutes Submission Form
- Faculty and staff only
- Use this form to upload meeting minutes for faculty or committee meetings
Office Hours and Syllabi Submission Form
- Faculty are required to submit their syllabi using the form linked above
- Faculty are required to hold one to two office hours a week depending on the number of credits they teach.
- If less than three credit hours, faculty should schedule one office hour per week.
- If faculty are teaching three credits or more, they should schedule a minimum of two office hours per week
Website Requests
Faculty/Staff Website Profile Form
- New faculty and staff can submit biographies and headshots to be added to DFPA faculty and staff website pages.
- Current faculty and staff can submit updates to the biographies and headshots as needed.
- Updates are made on a rolling basis.
- Faculty and staff only
- Request changes in the form of: correcting typos, updating outdated information, and bringing awareness to broken links.
Marketing & Communications
- Open to current DFPA faculty
- Please complete this form if you'd like your class added to the rester of courses prospective students can observe for 5-7 minutes during their DFPA tour. Prospective students will be escroted by their tour guide.
- Open to faculty, staff, current students of the DFPA, or alumni
- For items such as award nominations, publications, solo/group exhibitions, panel discussions, reviews, publications, and theatre productions.
- Items may be published on DFPA websites, social media channels, Sakai, email blasts, or internal listservs, depending on the nature of item.
- All news and announcements submitted are subject to approval and may be edited for style and clarity. Submission does not guarantee publication.
- Outside or third-party submissions are not accepted
